  • Health Insurance 

    • High Deductible Health Plan (HDHP) with Health Savings Account (HSA)  

    • OFC provides a contribution towards employee’s HSA 

    Dental Insurance

    • $1,000 maximum benefit per participant per year 

    • Benefits include preventive services, basic and major restorative and orthodontia  

    Vision Insurance 

    • Employee may purchase vision insurance for self, spouse and/or children 

    • Benefits include services for eye exams, frames, corrective lenses and contact lenses 

    Life & AD&D Insurance 

    • OFC provides term basic insurance & accidental death & dismemberment insurance   

    • Employees may purchase supplemental life & accidental death & dismemberment insurance  for self, spouse and/or children 

    Long-Term Disability Insurance 

    • OFC provides term basic insurance 60% of salary up to $15,000 per month 

    Flexible Spending Accounts 

    • Limited Health Care FSA - can be used to reimburse out-of-pocket vision and dental expenses 

    • Dependent Care FSA - reimburse expenses related to the care of eligible dependents
